1)  You know me, do I need to fill out a Permission Slip?

The short answer, Yes.  Every teen, without exception is required to have a year-long Parental Consent / Medical Release Form on file.  They will also be required to have a Simple Permission Slip for some special events and activities.  An explanation of the two types of permission slips is below.

2)  Why do you have Permission Slips?

We have permission slips for 3 basic reasons.  1)  For insurance and liability.  It is for the protection of the church, staff and volunteers.  2)  Because teens are more free they may assume they can attend a church activity without telling their parents where they are.  We believe that, though teens have more freedom, parents are still in authority over them and have the final say.  We want to be certain you know they are with us and have given them permission to come with us.  3)  The "just in case" reason.  If something did happen, though we pray it does not, we will have contact and medical information that may be required in an emergency. 

3)  What are the 2 types of Permission Slips?

We have 2 types so you don't have to fill out your medical information every time we need a permission slip.  Here is an explanation of the 2 types:

1:  Year-Long Parental Consent / Medical Release Form

This is a YEAR-long permission slip.  EVERY teen participating in youth activities is required to have this permission slip in our file.  It only needs to be filled out once during the year.  The entire paper, front and back, must be filled out.

2.  Simple Permission Slip

These are given out during the year for special events and activities.  It basically lets us know that YOU have given permission for your teen to attend a particular event.  We don't want them attempting to join us for a trip and you not knowing they are with us.
