Pay as Easy as 1-2-3-4

1)  Choose whether you will have a 3-member or 4-member team

2)  Enter your Team Name, Contact Phone Number & E-mail Address

3)  Click on the "Pay Now" Button

4)  Fill out all information, including Player's Names, Birthdates and T-shirt sizes (Sizes are Youth: S, M, L  and  Adult:  S, M, L, XL)
